To view your current Individual Evidence of Coverage and Schedule of Benefits, enter your Member ID number or the Policy ID number you received by email.

• Individual Evidence of Coverage – describes the details of your policy including services, supplies, exclusions and limitations.
• Schedule of Benefits – a document with a list of covered benefits and the member's cost share.

Note: Policy ID may also be referenced as Certificate ID in other correspondence.
